through the first decades of the 20th century, north carolinians fell victim to diseases like typhoid, hookworm, pellagra, and cholera. advances in science soon led to changes in sanitation, vaccination, regulation, and health education within communities,

N. C. State Health Plan, all of which lowered infection rates. in 1911, guilford county opened the second full-time county health department in the united states. a year later, robeson county established the first rural county


health department in the country. other counties followed suit, with 11 offices in operation by 1914. to improve the quality of efforts across the state, dr. watson smith rankin, secretary of the state board of health, outlined a partnership model: costs and oversight were to be shared by both counties and the state. the rockefeller foundation was brought in to help create a county health program focused on teaching, training, and service. in 1917, wilson county was the first to pilot the program


under the leadership of benjamin earle washburn. in its first year, nearly every home received sanitation education. typhoid vaccine was offered to all, with strong support from local leaders. the results were significant: the average death rate from typhoid fever in the county decreased 58 percent in a single year. the success drew state and national attention, and the general assembly expanded the model to ten additional counties.


by 1919, 45 percent of north carolina's population was served through local health departments — the highest percentage in the nation. by 1923, the state's death rate was half of what it had been in 1911, the year guilford county's health office opened. by 1949, north carolina led the nation in county health work — all 100 counties had full-time health departments. this model of cooperative county health work was adopted by many southern states, and still stands as an inspiration.
